Camper Forms

Please find the downloadable 2025 Health and Physical Exam Forms below,
in addition to the online Scholarship Application.

Health Forms must be completed for each camper for the 2025 season and uploaded on our registration site.

Physical Exams should be submitted for each camper for the 2025 season. There are a few ways you can do this:

• Download our form here to be filled out by a registered medical provider.

• Submit another health exam/physical that has been done within 24 months of camp attendance dates. This can be a physical form for another program, school, sports, or even a detailed visit summary from your camper’s last doctors visit approving them for physical activities as long as it has been completed by a registered medical provider.

• Locate your physical exam form from last summer. These will be accepted if they are within 24 months of the camp program and should be re-uploaded on the registration site account.

We recognize that getting in for an annual physical may be a challenge, because of this if you are not able to get one of the above physical exam forms completed by June 1, please let us know.
